Licensed Practical Nurse (LPN) needed to provide direct and indirect nursing care aligned with physician orders and resident care plans.

• Conduct resident reviews upon admission, monitor ongoing health status, and document findings in electronic records.
• Update care plans with resident status changes and communicate updates to staff.
• Administer medications, treatments, and procedures as prescribed, and document accordingly.
• Respond to and document resident and family concerns and actions taken.
• Perform routine rounds to ensure residents' safety, comfort, and care compliance.
• Assist with activities of daily living and supervise direct care staff as needed.
• Requirements include current LPN licensure, CPR certification or willingness to obtain, strong communication skills, leadership abilities, basic computer skills, and a passion for geriatric care.
