• Provide care planning and assist with resident assessments for those needing advanced care.
• Complete all required documentation promptly and accurately.
• Collaborate with interdisciplinary teams to ensure proper resident care and communicate changes in condition.
• Write summaries to support physician care and perform initial assessments for new admissions and hospital returnees.
• Make rounds to observe residents' conditions firsthand.
• Qualifications include a valid PA RN license, CPR/AED certification, and preferred two years' experience in institutional care.
