This RN Case Manager role offers up to a $15,000 sign-on bonus (restrictions may apply).

• Provides direct patient care, develops and documents individualized care plans based on assessments and OASIS evaluations.
• Coordinates home care services, supervises LPNs and home health aides, ensuring quality and safety standards.
• Conducts home visits primarily in Onondaga County, with occasional visits to surrounding areas; schedule is Monday-Friday 8a-4:30p, with one weekend/month, no on-call.
• Requires NY RN license (BSN a plus), a valid driver’s license, and proficiency in electronic medical records.
• Physical demands include lifting, patient transfers, and driving in all weather conditions.
