We are seeking compassionate Registered Nurses (RNs) for full-time Hospice case management in Greater Detroit.

• Assess and evaluate patient needs, develop care plans, and coordinate services across healthcare teams.
• Serve as a liaison between patients, families, and providers, ensuring effective communication and understanding.
• Monitor patient progress, adjust care plans as needed, and educate families on health management.
• Maintain accurate documentation following HIPAA regulations and participate in quality improvement initiatives.
  
  

Qualifications include a minimum of 1-year nursing experience, active RN licensure, CPR certification, and a valid driver’s license with auto insurance.
