We are hiring an RN Case Manager to oversee care coordination for Medicare Advantage members.

• Responsibilities include managing patient care plans, conducting assessments, coordinating with healthcare teams, and providing patient and staff education.
• Visit patients in person or telephonically, review medical records, and ensure proper care and documentation.
• Manage a caseload, participate in audits, and support discharge planning.
• Qualifications include a valid RN license, minimum 2 years of geriatric or skilled nursing experience, and 2 years in managed care.
• Preferred certifications include CCM.
