Seeking an experienced Home Health Occupational Therapist (OT) for an ongoing, per diem role in Reading, PA, serving adult and geriatric patients in their homes.

• Perform home visits, evaluate functional status, and develop personalized intervention plans.
• Assess cognitive, psychological, motor skills, and home environment hazards.
• Identify equipment needs, educate patients and families, and document care using electronic records.
